Job DetailsLevel: ExperiencedJob Location: Winslow Indian Health Care Center - Winslow, AZ 86047Position Type: Full-TimeEducation Level: Bachelors DegreeTravel Percentage: OccasionallyJob Shift: DayUnder general supervision of the Youth Wellness Manager and indirectly the Director of Hozhoogo Iina Wellness, provides case management, care coordination and maintains clinical standards of care for pediatric patients who are high risk for developing diabetes, and pediatric patients living with diabetes and other co-morbidities. Coordinates the pediatric wellness clinic (Victory) and coordinates appropriate management of pediatric wellness referrals. Provides nursing support through planning, assessment, implementation, evaluation, education and intervention among children, families, the schools and the communities of Winslow service area with the goal of preventing diabetes and other chronic disease illnesses such as obesity.
