Job description

When in the Nurse Navigator portion of the job: - Acts as a liaison between the patient, their families, and internal and external providers. - Establishes a strong rapport with patients, families and other members of the health care team and community. - Assists with the implementation of a treatment plan for patients being discharged from the department. - Facilitates appointment scheduling, referral process and providing clear discharge education to the patient and family when appropriate. - Recognizes barriers to healthcare and connects patients to the resources, services, and system providers during discharge. - Communicates with family members (next of kin) of impending patient discharges to expedite transport back to a facility or home. - Facilitates external transport to assist with more timely transport back to facilities. - Collaborates with ED Ambassadors, Patient Liaisons and the multidisciplinary team on Patient and Family Communication and Experience. - Daily review and triage of all boarding patients in the department, coordinating bed needs by hours boarding and clinical needs. - Daily review of our geriatric population as well as our medically compromised patients...
