Default Work Shift: Day (United States of America) Hours: 40 Salary range: $37.19 - $56.49 Schedule: Full Time Shift Hours: 10 Hour employee Department: Case Management Social Services Job Objective: Coordinates the complex discharge planning needs of patients and their families, as well as to provide supportive counseling, psychosocial assessment and interventions for patients with complex psychiatric, social, medical and financial needs.
