Job description
This role focuses on supporting hospice patients and their families through serious illness, end-of-life care, and bereavement. In addition, this role performs specialized hospice social work duties, including psychosocial assessment, counseling, goals-of-care support, advance care planning, and coordination with community resources as a member of the interdisciplinary hospice team. Responsibilities also include identifying and addressing psychosocial, emotional, caregiver, financial, cultural, and practical needs related to terminal illness and decline; facilitating coping and adjustment; providing grief and bereavement support; assisting with care transitions and discharge planning; and advocating for patient autonomy, dignity, comfort, and access to appropriate home and community-based services. This is a professional individual contributor role that may guide lower-level staff or support program processes. The majority of time is spent delivering and coordinating specialized hospice social work services using advanced clinical knowledge and sound judgment.
