Job Title: Social Worker - Comp Psych Emerg Prog Cpep - Mount Sinai West - Full Time - Day (Tuesday-Saturday: 10am-6pm) - Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. - Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned. - Develop options and treatment plans in a timely manner. - Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals. - Ability to identify, select, access and make referrals to appropriate community resources. - Monitor, coordinate, manage activities for high risk psychosocial patients to ensure implementation of multiple services. - Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies. - Identify and assist team in managing obstacles to treatment and/or discharge plans. - Provide psycho-education for patients and families. - Knowledge of child protection, elder abuse, domestic violence policies and protocols. - Participate interdisciplinary rounds, case conferences,...
