3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city’s few Youth Access Points. The Residential Behavioral Health Program spans two sites: the Lower Polk TAY Navigation Center and Just Home Transitional Housing. Together, these programs provide behavioral health and supportive services to Transitional Age Youth (TAY) experiencing homelessness and system involvement, with a focus on stabilization, skill-building, and long-term housing readiness. The Lower Polk TAY Navigation Center (TAY Nav) is a low-barrier residential shelter serving unhoused TAY. The program is grounded in a “meet youth where they are” approach, prioritizing safety, accessibility, and engagement.
