Position Title Rounding Nurse Olathe Hospital: Position Summary / Career Interest: Responsible for the delivery of professional nursing services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction, and coordinates discharge planning with other disciplines. Responsibilities and Essential Job Functions: - Supports daily interdisciplinary rounds through real‑time documentation, care board updates, and escalation of care barriers. - Supports safe, timely discharges by managing orders, identifying needs, and collaborating with interdisciplinary partners. - Validates that patients have clear and concise information and understanding of discharge instructions - Identifies and prioritizes patient and family learning needs across diagnoses, medications, procedures, and discharge instructions. - Facilitates interdisciplinary communication by coordinating updates, clarifying care plans, and supporting collaboration across clinical teams. - Promotes quality and safety by monitoring patient needs, escalating concerns, and supporting org...
