The Care Coordination Supervisor directs the programmatic supervision of care coordinator teams and ensures the implementation of the specific program model. The supervisor is primarily responsible for ensuring adherence to high fidelity wraparound model of care coordination, ensuring children, youth and family/caregiver centered, high quality services, positive outcomes, and satisfactory business results for the program serving children and families/caregivers of a variety of socioeconomic, racial, ethnic, and cultural backgrounds.
