Geriatric Care Manager - Licensed Social Worker or Registered Nurse

Mint Care Management at The Senior Company is seeking an experienced Registered Nurse (RN) or Licensed Social Worker (LSW/LCSW) to join our geriatric care management team serving older adults and their families throughout New Jersey. This role focuses on providing high-touch, personalized support to aging clients and their families, many of whom live out of state and rely on us as their trusted local advocate. Our care managers help families navigate complex medical, social, and care planning decisions while ensuring clients receive the highest quality care and support. Key Responsibilities: - Conduct comprehensive in-home assessments - Develop and oversee individualized care plans - Coordinate and monitor medical care, home care, and community services - Accompany clients to medical appointments and advocate when needed - Provide ongoing support, guidance, and education to families - Assist with care transitions (hospital, rehab, assisted living, memory care) - Maintain consistent communication with families and care teams - Serve as a trusted liaison between families, providers, and caregivers Qualifications: - RN, LSW, or LCSW license required (New Jersey) - Experience working w...
