Description First Choice Hospice (Jet Health, Inc.) is looking for a full-time Licensed Practical Nurse (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving hospice services in the Treasure Valley area. The schedule for this position will be Tuesday-Saturday. The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes. Requirements: - Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse. - Coordinate care with the interdisciplinary group, patient/family, and referring agency. - Assist in the development and implementation of patient care plans. - Monitor patient health and report any changes to the supervising RN. - Administer medications and treatments as prescribed. - Educate patients and their families on health care needs, conditions, and options. - Maintain accurate and timely documentation of patient care and progress.
