Job DetailsJob Location: Plymouth, Ma - Plymouth , MA 02360Position Type: Full TimeSalary Range: $22.75 - $22.75 HourlyRTR is offering a $2,500 Sign on Bonus for New Full Time Hires! Support and evaluate adults with developmental disabilities in a day habilitation setting. Member of an interdisciplinary team in the coordination of therapeutic services. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Review each recipient’s treatment and habilitation goals monthly and inform staff of any charges in the recipient’s status. Supervise the implementation of the ISP/DHSP and maintain the individual’s records. Coordinate OT, PT, Speech, Nursing and behavioral services for the assigned caseload of individual members. Case Management responsibilities include writing progress notes, writing Individual Support Plans and Day Habilitation Service Plans, writing assessments and objectives for annual review. Assist with all development of behavioral plans or other intervention strategies. Chart data collected from programs.
