Children’s Care Coordinator Children’s Behavioral Health Home (CBHH)- South Portland Middle School The Children’s Care Coordinator is a direct service staff position for the Children’s Behavioral Health Home program. This is a unique opportunity and partnership with South Portland Middle School to provide care coordination to students in the community school setting. This is a team-based case management model to include working with a peer family support specialist, nurse care manager, and medical/psychiatric consultants to provide holistic care coordination for children and youth. We offer a high level of supervision and support, to include individual and group supervision. There is no on-call, night, or weekend expectation. It is important to us that there is high quality work happening for families, so having work/ life balance is crucial and the supervisors will ensure the workload feels manageable.
