Our Company All Ways Caring HomeCare Overview: The Registered Nurse oversees, and coordinates patient care delivered in the home environment. This role blends clinical nursing expertise with care coordination, patient advocacy, and ongoing assessment to ensure patients receive safe, effective, and person‑centered care at home. The Registered Nurse works closely with caregivers, physicians, therapists, and community partners to support optimal health outcomes and reduce avoidable hospitalizations. External Job Description: - Documents nursing actions of all care given to individuals and informs physicians and family accordingly - Conducts nursing assessment visit as needed - Follows policies and procedures according to operation, state and regulatory standards - Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation - Implements nursing support for individuals in a home setting - Works with Director or Nursing/Nurse Supervisor and/or Branch Manager to address the health needs of individuals - Participates in staff meetings, in-services and other meetings as needed -...
