Description SCHEDULE: Part-Time: 30 hours per week between the hours of Monday-Friday 8:00am-4:30pm SUMMARY: Promotes the ability of high-risk individuals in the community-based housing programs to achieve independent living skills and stability in their psychiatric or medical conditions. Case manager works with individual to maintain housing and prevent homelessness. This includes tenant advocacy, apartment inspections, and finding new apartments as needed. Case/care management links individuals to service systems, then coordinates and monitors the provision of services. Services are individualized to the specific identified needs of each person in a culturally sensitive way. This is achieved using a holistic approach considering several areas of strengths and needs including, but not limited to the individual’s: history, culture, spiritual preferences, education and employment information, social involvement, mental health and substance abuse status, medical needs, housing advocacy and needs, safety issues, legal issues and financial issues.
