About the Role: The purpose of this role is to drive a whole of service system approach to deliver person centred outcomes in the areas that Link Wentworth operate. Using a case management framework, you will assess tenants' support needs, strengths, and support networks to determine their goals, develop case plans, and refer to services to sustain tenancies and increase tenant health and wellbeing. - Full time (35 hour work week) - Ongoing Opportunity - Based in West Ryde with some local travel required during work hours in the Greater Sydney area Key areas of responsibility include: - Identifying, assessing, and responding to risks that may impact tenant safety, wellbeing, or the sustainability of their tenancy, including risks to the organisation.
