When you join our team, you’ll receive: Position Summary We’re seeking Full-Time Case Managers to join our dynamic healthcare team. The Case Manager participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.
