What You’ll Do: As a private duty nurse on our home health team, you’ll care for medically complex patients, primarily children , where they are most comfortable, at home. - Provide personalized nursing care for pediatric and/or adult patients with complex needs - Coordinate care plans with physicians, therapists, and other team members - Monitor patient progress and update care plans as needed - Educate and support families with in-home care techniques - Document visits and care notes in our easy-to-use EMR (electronic charting system) - Collaborate with your clinical supervisor and participate in case conferences - Be a trusted part of a long-term care team focused on safety, comfo...
