Market Range: 05 Hiring Salary: $18.54/Hourly: THIS IS A GRANT-FUNDED POSITION FUNDED UNTIL DECEMBER 31, 2026. JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Health Coach assists with health screening and health coaching using motivational interviewing techniques to support participants at the Health Hubs in the neighborhood Health Hub program. This position collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports. Establishes trusting relationships with participants to encourage engagement with individualized programs and healthcare providers for improved health outcomes. Assesses and addresses barriers to access preventive care. Collaborates with medical professionals and program leads to create educational materials for participants. Educates/coaches program participants on disease prevention and advocacy for improved health care. Coordinates services with internal and external behavioral and mental healthcare providers.
