Our Care Partners are supported by modern systems, streamlined processes and strong operational support, helping reduce administrative pressure so you can focus on building relationships and delivering great client outcomes. Due to continued growth, we’re looking for a Care Partner to join our team on a permanent full-time basis, based from our Carrum Downs office with hybrid flexibility. The Role: You’ll support a portfolio of clients receiving Support at Home / Home Care Packages, helping them remain safe, independent and connected in their own homes. Key responsibilities include: - Conducting client assessments and developing personalised care plans - Coordinating services with healthcare professionals, support workers and families - Monitoring and reviewing care plans as client needs change - Supporting clients and families through their Support at Home journey - Maintaining accurate documentation and meeting compliance requirements About You: You may already...
