Position Summary: The Transition Specialist is responsible for comprehensive, culturally competent services to individuals detained in the city jails and upon reentry to IMPACT. What You'll Do: Case Management: Provide individualized services to 100% of Participants on caseload from the point of orientation to exit. - Conduct thorough assessments and, through a motivational interviewing process, develop individualized person-centered Comprehensive Service plans for 100% of participants on caseload. - Provide crisis intervention and engage in safety planning. - Provide evidence-based clinical support as needed. - Facilitate care from Rikers through successful re-entry in the community. This includes ensuring that connections are made to appropriate internal and external services as required to meet the needs of the participants. - Work in conjunction with Reentry Mentors and Reentry Coordinator to ensure seamless transition and continuity of caregiver for 100% of participants on caseload.
