Overview Location: Naval Medical Center Portsmouth, VA Hours of Performance: Services shall be provided Monday through Friday, between the hours of 0700 and 1600 hours. Collaborate with other health care disciplines toprovide total quality patient care. Communication avenues may include, but not all inclusive ofasynchronous messaging to improve communication, telephone consultations and other nontraditional means.-Function as care coordinator to assure all aspects of patient care needs are met, to includeassessment, intervention and follow-up in screening of patient/patient problems.-...
