Location: 100 Arrow Springs Boulevard Lebanon, OH 45036: Work Schedule: Clinic hours are 7:15am-6:00pm Monday, Tuesday and Thursday, 8-4:30pm on Wednesday and 7:15 to 3:45 pm Friday.
