Overview Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment. The Continuing Care Coordinator will work on a Full-Time basis with our Meadows CCBHC Open Access Program. The schedule will be Monday through Friday, 7:30am-4:00pm. In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500! Pay Rate: $20.00 - $25.00 per hour. Responsibilities: - Coordination of continued treatment and/or care of patients leaving services with internal and external program and/or resources. - Development of aftercare planning in collaboration with the patient and any significant other(s) the patient chooses to involve. - Assisting with the physical discharge process. - Explaining and presenting resources within the community that will aid in the patients recovery process once they leave the program. - Coordination of care with referral sources. - Case manage...
