Position: Care Partner - Support at Home Location: Across NCN Health: EFT/Hours: 1.2 EFT (Multiple positions available) Status: Permanent Part Time: Hourly Rate: Depending on qualifications and previous experience plus 12% superannuation About the Role: NCN Health is seeking a Case Partner to join our Support at Home Program team. The Care Partner supports older people to live safely and independently at home through person-centered care under the Support at Home Program. They coordinate services, explain the program, and help participants understand their goals, budgets, and support options. The role involves monitoring care plans and budgets, promoting independence, and addressing risks and changing needs. Working within the NCN Health team, the Care Partner ensures continuity of care, quality improvement, and regulatory compliance. About You: - Vaccination against seasonal Influenza (mandatory) - Vaccination against Covid-19 (recommended) - A relevant post-secondary qualification in an appropriate Health and/or Human Services practice, including Community Services, Aged Care, Nursing, Allied Health, Social Work, Care Coordination. - Demonstrated experience in assessment, care ma...
