Employment Type: Part time Shift: Description: ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions. Assesses, plans, evaluates, and coordinates management of patient care including discharge planning and utilization needs . Supports ministry and system goals for capacity management through achieving length of stay goals, identification of avoidable delays at department approved levels, and other Case Management initiatives. Coordinates with the interprofessional team in meeting patient care goals . Establishes partnerships with site Patient Experience champions to align ef forts with patient satisfaction outcomes. Identifies discharge needs, determines the appropriate level of care that the patient can transition to, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care and prevent the patient from having to readmit to the hospital.
