The Care Manager will work in the Adult Foster Care (AFC) department where it is a great opportunity to work in a unique program. Care Managers make a truly wonderful difference in the lives of children and adults with a wide range of disabilities. Essential Functions: ▪ Conduct an initial biopsychosocial assessment and evaluation of a member’s appropriateness for AFC, with ongoing monitoring as needed: ◦ Interview participants for the AFC services with empathy to problems such as personal and family adjustments, finances, employment, food, clothing, housing and physical and mental impairments to determine nature and degree of problem and recommends to AFC program team admission and individual goals. ◦ Secure information to complete referral process administrative requirements ◦ Subm...
