About this opportunity As a Care Integration Nurse or Paramedic, you will collaborate as part of a broader team working closely with Medical Officers, Nurse Care Coordinators, our service provider network and the Local Health Networks (LHNs). You will be responsible for delivering patient-centred care, to suitable patients for Amplar Health Home Hospital (AHHH) and undertaking clinical reviews as part of a multi-disciplinary team. AHHH is committed to providing safe, high quality and effective health care services that address the holistic (physical, social, psychological, and spiritual) needs of our patients and their families and carers. Our design of the AHHH model has been informed by core principles that embody the change that we want to see (and create) in acute care. This role will involve undertaking visits in the community to our patients’ homes delivering nursing care as required by their care plan. You will have use of our company vehicle for patient visits starting your shift from our office in Eastwood.
