Our Company Adoration Home Health and Hospice: Overview: Responsible for coordinating all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources as well as staff. Responds to customer requests and concerns, works closely with Authorization Specialist to manage the insurance verification processes and assist with the authorization processes. Communicates scheduling changes between office and field staff accurately and timely. Responsibilities: - Receive and process new patients for admission to our agency using EMR system. • Coordinates care by assigning case manager and clinician(s) to ensure 48-hour admission.• Communicates with various healthcare professionals to provide highest level of care.• Use HIPAA compliant processes to receive and process orders/faxes for new and current patients.• Performs follow up calls to patients and caregivers to offer clear and supportive communication.• Performs other various coordination duties to support day to day work flow.• Maintains positive attitude when dealing with others.• Organized and able to multi task, with meticulous documentation.• Goal oriented, proactive and...
