Job Summary Under the supervision of the Director of Activities provides MDS documentation, Resident assessment of current interests and activity participation, and care planning. Education & Experience: - Previous experience with the elderly - Formal High School Diploma or equivalent - Minimum of one year of Activities experience required, i,e. documenting Resident care planning, developing and implementing new activities. Experience Desired: - Ability to play musical instrument - Knowledge of art - Knowledge of MDS Skill Requirements: - Ability to lead and motivate groups of Residents Essential Functions: - Documents the Residents initial assessment, interim assessments integrated progress notes, and Resident care - Participates in team conferences and care planning - Documents residents progress and Interaction during leisure time - Develops Resident care plans tor Resident activity programs - Directs and leads assigned activities; participates in off premise trips and events - Modifies activities and/or equipment to facilitate participation in activities for Residents with disabilities - Assists Residents on assigned units with activities of interest - Guides volunteers who are...
