Come work at the best place to give and receive care! ​Job Description: About the Job: The Recovery Case Manager is responsible for providing casework services to clients enrolled in the Recovery Court program. The Case Manager Provides clinical case management interventions with patients and families that address personal and environmental issues to maximize emotional, social and physical wellbeing and the appropriate and efficient use of health care, substance use and mental health services across the continuum. Promotes efficient coordination of patient care through collaborative practice with the Program team. Coordinates patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care. What You’ll Do: - Collaborates and participates with the multidisciplinary team in the ongoing clinical assessment of patient’s status/care management - Provides case management services in assisting the patient in obtaining housing, substance use treatment, employment, insurance benefits and any other needs - Monitors patient’s progress to ensure care is appropriate and timely - Coordinates transportation for clients to access SUD...
