At Westside Family Healthcare, the Home Visiting Community Health Workers (HV-CHW) helps patients, and their families navigate and access community services and other resources to adopt healthy behaviors. The HV-CHW provides health promotion information during the stages of prenatal and up to 5 years of age, service referrals to home visiting services, and case management patients and members of the community.
