Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm General Summary: Supervises and oversees the work activities of Care Management social work and social services staff and works in a team leadership model to develop and maintain departmental operations that are consistent health system, ambulatory, inpatient, and community needs. May carry a caseload and provides services to patients and families including care coordination across the continuum, transitions of care support, and referral to community services. Essential Functions: - Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions. - Assists in developing budgets. - Identifies needs in program planning and facilitates program development. Supervises patient care, family communication, and record keeping. - Collaborates with inpatient, ambulatory, population health, and community programs, as well as external agencies and payer partners, to support coordinated patient care across the continuum. - Provides direct service to patients including psychosocial assessment, interviewing patients and fami...
