Job Title: Social Worker - Continuing Health Care (CHC) Salary: £39,027 – £44,463 per annum (pro rata if part-time) Grade: E Contract Type: Fixed Term - 24 months Working Hours: 37 hours per week Thurrock Council are looking for a qualified Social Worker to work within our Continuing Healthcare (CHC) Adult Social Care (ASC) Team under a fixed two-year contract. As a registered and qualified Social Worker , you will be working within a multi-disciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You will manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities. About the Role: The role involves working within Adult Services to undertake Care Act assessments and reviews for those with identified complex needs with potential eligibility for CHC.
