JOB TITLE : Registered Nurse Care Coordinator Key Tasks and Responsibilities: - Perform nurse visits in the clinic, including vitals, medication reviews, perform pill counts, post‑procedure checks, follow‑up assessments, and patient education. - Serve as the primary nursing point of contact for patients requiring ongoing care coordination. - Assist with patient assessments, medication reconciliation, and review of medical histories. - Monitor treatment response, side effects, complications, and care plan adherence. - Coordinate referrals, imaging, diagnostic testing, and specialty care. - Collaborate with providers to develop, implement, and modify individualized pain management care plans. - Educate patients on pain conditions, medications, safe use of controlled substances and interventional procedures. - Promote patient self‑management strategies and shared decision‑making. - Process medication refills in accordance with clinic protocols, provider guidance, and regulatory requirements. - Assist physicians with intrathecal pump refills. - Maintain accurate and timely documentation in the EMR. - Communicate effectively with providers, MAs, schedulers, and external partners. - Part...
