Description We are seeking a compassionate and knowledgeable Case Manager (Mid-Level) to provide transition and career support services for our program participants. This role will involve offering career guidance, mentoring, and coaching to aid successful reentry into the workforce and ensure employment retention. The Case Manager will conduct follow-up services for at least one year post-release to support job retention and career progression.
