Department: 39110 Waukesha Jefferson - Home Health Aides Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: M- F flexible schedule Pay Range: $20.80 - $31.20 Major Responsibilities: - Contributes to effective patient outcomes. Responsible to provide competent patient care assigned. - 1)Demonstrates competent home health aide skills. - 2)Promotes patients to reach functional status goals by providing assistance with ADL’s following the HHA assignment record in rendering patient care and maintaining safety. - 3)Notes changes in patient’s condition requiring further attention, informs the case manager, or the RN/Supervisor and documents accordingly. - 4)Understands the hospice philosophy, comfortable providing specialized care to the terminally ill. - 5)Performs personal care needs, feeding patient, laundry, light cleaning of patient room and washing. - 6)Meeting safety needs of patient using equipment safely and properly. - 7)Taking temperatures, pulse, respiration and blood pressure as directed by the home health aide assignment sheet when assigned. - 8)Provides skin care and changing patient position to prevent breakdown as assigne...
