Employment Type: Full time Shift: Description: Status: Full time Position Purpose: Provide occupational therapy evaluation and treatment for participants in PACE programs, nursing homes, or in-home settings. Position Details: This position will work both in the clinic and complete home care assignments as well. Shift time could also flex to 830 - 5 pm if desired. What you will do: Perform in-person comprehensive occupational therapy assessment for initial acceptance, including problem identification and care planning. Conduct assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety. Provide treatment to participants and implement occupational therapy procedures consistent in achieving identified goals, grade activities to facilitate goals, and confer with the Interdisciplinary Team on participants' care issues. Assist in the ordering, inventory, distribution and maintenance of durable medical equipment to participants.
