Find your purpose at CentraCare as an Addiction Counselor at St. Cloud Hospital. In this role, you will provide assessments, counseling, and care coordination for patients experiencing substance use disorders while collaborating with an interdisciplinary treatment team to support recovery and long-term wellness. Position Responsibilities: - Facilitate psychosocial and chemical dependency assessments. - Develop and implement individualized treatment plans and therapeutic interventions. - Provides clinical interventions to include individual, couples, family, and group education to inpatients/outpatients ages 12 years to geriatric (depending on site) - Coordinate discharge planning and community resource referrals. - Provide case management and collaborate with interdisciplinary team members. - Serve as a liaison between treatment teams and referral or consulting resources. - Provide consultative services to other behavioral health sites, including Telehealth assessments as needed. - Support patient safety practices and Family Centered Care principles.
