Case Manager with DMH Children, Youth, and Family (CYF) 26000353 Description The Metro Boston Area Child, Youth, and Family Services Team operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will provide Case Management services for eligible youth up to the age of 22, with the majority of caseload below age 18. The successful candidate will have knowledge and desire to work with youth with significant mental health needs and their families to promote recovery and connection to community-based services. As Metro Boston Area CYF staff, the Case Manager will provide assessment, outreach, support, and service coordination to DMH authorized individuals by promoting DMH’s philosophy of recovery, trauma informed care, person, youth and family-centered choices. Please Note: The Work Schedule is Monday through Friday 8:30am to 5:00pm (40 hours/week) with Saturday and Sundays off.
