Job DetailsJob Location: Valir Hospice - Kingfisher - Kingfisher, OK 73750Position Type: Full TimeTravel Percentage: Up to 25%Job Shift: GraveyardJob Category: NurseShift is FRIDAY 5:00 PM - MONDAY 8:00 AM. This is a full-time position in the Kingfisher area. Travel: Up to 100% RN Duties/Responsibilities: Oversees patient cases from admission to death/discharge. Completes all nursing assessments for each patient assigned. Responsible for maintaining patient medication profiles, updating as needed, while ensuring updated profile is in patient charts at the office, as well as in the facility or home. Updates the plan of care for each assigned patient every two weeks. Completes the re-certification process by assessing and accurately documenting findings on appropriate forms. Makes RN supervisory visits every 14 days on all assigned patients. Completes admissions as assigned. Provides on-call hours as determined by Patient Care Manager. Accurately communicates patients’ information to Patient Care Manager and medical staff.
