Core Responsibilities: - Provide medication management to individuals including but not limited to medication observation, treatment and monitoring of side effects, monitoring and supportive services to increase compliance - Inform Psychiatrist, Nurse Practitioner (NP), Primary Care Physician (PCP), Case Manager, and mental health provider, as appropriate, of medication concerns/issues - Provide case management by coordinating and implementing mental health and recovery support services with individual members - Document all services provided in clinically appropriate progress notes with timeliness and accuracy - Coordinate client services and appointments by work...
