Job Summary This is a full-time professional position. This position provides the after hours nursing coverage for patients on assigned weekends, holidays and week nights to provide after hours nursing support to patients and families who have questions, concerns or emergencies. This position requires staff to make visits to patients’ homes to provide symptom relief and or support at the time of a patient’s death. This position collaborates with the manager on call and the weekend on call triage nurse. The schedule for this position is Thursday-Sunday 10p-8a. Responsibilities what you do: - Plan, provide, supervise and document safe, competent, and compassionate hospice nursing care for patients in accordance with physician orders and established policies and procedures. - Provide after hours clinical and emotional support for patients and families in crisis. - Assist the triage nurse, when needed, in determining and completing appropriate coverage of after-hours phone calls from patients and families. - Responsible for current knowledge of nursing practices. - Responsible for providing, and documenting, appropriate and relevant end of life information and education to patients and...
