Job description

The Community Health Registered Nurse (PD RN) will implement the Community Health department’s clinical and outreach initiatives, connecting and engaging with patients and community members through health and wellness events, screenings, flu shot clinics, and education programs. This role provides direct clinical support for community-based health services, assists patients in navigating care and resources. Provide clinical oversight at community-based health events, including blood pressure, glucose, cholesterol, and other health screenings and flu shot clinics; ensure safe practices and adherence to AHS protocols. Educate community members on preventive health, chronic disease management, and healthy lifestyle behaviors using evidence-based practices and culturally appropriate communication. Support care coordination by identifying abnormal results, providing health counseling, and facilitating referrals or follow-up care through collaboration with AHS clinical departments and community agencies.
