Description Position Overview: As a Case Manager II at a in-custody (jails) program , your role will involve providing comprehensive support services to individuals who are incarcerated, You will be responsible for assessing participants' needs, developing individualized service plans, and connecting them with appropriate resources to help them regain stability and move towards permanent housing. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency. Duties and Responsibilities: Client Assessment: Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals. Case Planning: Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs, such as housing, employment, healthcare, and social support. Referrals and Resources: Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance.
