Provides psycho-social evaluation for patients with Chronic Renal Failure. Provides ongoing assessment and interventions for their patients and families. Advocates for patients. Required Qualifications: - Master's Degree - Upon hire: California Clinical Social Worker License. - 4 years experience in a mental health setting. - 1 year experience in outpatient dialysis setting. 4 years preferred. - 4 years' experience in a position that required participation on an Interdisciplinary Treatment Team. Preferred Qualifications: - Upon hire: CPR Certification.
