Sun River Health is seeking a full-time Integrated Support Care Manager to join our team at the Shirley location. Summary: Provides integrated care management services to patients using a harm reduction approach to address psychosocial barriers to care. Provides coordination of care, psychoeducation, casework counseling, linkage to community supports, referral follow up, and maintains communication with the patient and care teams. Attends ongoing training to address needs of patients including understanding of HIV, Substance Use, Hepatitis C, mental health, and chronic conditions. Performs job duties in compliance with systems and job specific standards.
