Description POSITION OVERVIEW: Improving the quality of life through prescribed exercise, hands-on care, and patient education, by examining each person and then developing a treatment plan to improve their ability to move, reduce or manage pain, restore function, and prevent disability. ESSENTIAL FUNCTION: - Performs and documents daily assessments of patient’s condition - Follows a plan of care which is appropriate to problems in identified and involves the patient/family according to their capabilities and desires - Educates the patient and family/caregiver about patient deficits. Plans and/or assists with patient discharge from therapy services and treatment settings. Provides information regarding appropriate selection/use of adaptive equipment and support programs. - Within scope of job, requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
