Summary: The Social Services Case Specialist develops, coordinates and delivers all direct case specialist services to patients and their families assigned to him/her, and performs other duties as directed. Job Duties & Responsibilities: Provide social services to patients and their families/significant others: Complete assignment and documentation within required time. Communicate professionally and accurately. Be professional. - Provide pre-admission consultation to community agencies, families, or patients. - Conduct social services portion of Multi-Disciplinary Assessment Tool within 24 hours of admission. - Obtain necessary consents for release of information as required by Hospital policy. - Provide treatment services to the individual, group and family as required by program guidelines. - Participate/lead patient/family/significant others education sessions. - Implement goals, objectives and interventions as specified on the Master Treatment Plan within the therapeutic component of the patient’s program. - Use knowledge and skills to collect and review information regarding the patient’s status. - Identify age-specific needs. - Provide interventions that are appropriate to t...
