SUMMARY: As an Outreach Worker, you greatly assist community members by providing meaningful interactions and connections to a variety of community-based resources and programs. STATUS: Part time (24 hrs.): LOCATION: Newark-Wayne Community Hospital - 1200 Driving Park Avenue Newark, NY 14513 DEPARTMENT: Hospital Operations: SCHEDULE: Monday - Friday 9:00 am - 3:00 pm ATTRIBUTES: Two (2) years of experience required Strong computer and interpersonal and communication skills Superior customer service and empathetic attitude RESPONSIBILITIES: Patient Care. Demonstrate knowledge of community resources and how to access them; complete initial assessment for new, eligible participants and collaborate with other care providers regarding participant needs; provide referrals for participants and their families; make home visits as needed; advocate for patients to ensure adequate housing, treatment, benefits and community services; develop patient treatment plan Documentation.
