Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. Required Qualifications: - Upon hire: California Registered Nurse License - 2 years Acute care clinical or comparative experience. Preferred Qualifications: - Bachelor's Degree in Nursing.
